The Bible teaches that a pastor/elder must have a profound understanding of the gospel of grace and is himself a beautiful witness (Titus 1:6-9; 1 Timothy 3:1-7). This form will help us assess whether you are called by God to be a pastor/elder. Please candidly and honestly answer the questions listed below. If you wish to express other thoughts than what these questions ask, or if you prefer to answer the questions through a phone interview, please contact the director of the examination department.

A pastor/elder must believe that all Scripture is God-breathed and is the true words of God. He must believe that all men are sinners and will face the eternal judgment; that by grace we have been saved through believing in Jesus Christ, who shed blood for us; and that only those who have faith in Jesus Christ will be saved. Do you fulfill these requirements? Please explain how.

A pastor/elder must be experienced and gifted in teaching the Bible, leading people to faith in the Lord and training disciples. Do you fulfill these requirements? Please explain how.

A pastor/elder must have a steadfast devotional life, know that God is his Father in Heaven and trust in the Lord in all things. Do you fulfill these requirements? Please explain how.

A pastor/elder must be a good husband and father and have a loving, God-centered family. Do you fulfill these requirements? Please explain how.

做為一位牧師（長老）必須是誠實的、謙虛的、有憐憫的、樂於寬恕和溫柔的；他必須不是有苦毒的、易怒的、或很快與人爭吵的。他必須能自守，並且不被酒、香煙、色情、金錢或任何有傷害的事所捆綁。您是否符合此條件？請解釋。
A pastor/elder must be honest, humble, merciful, ready to forgive and gentle. He must not be bitter, irritable or prone to quarrels. He must be self-disciplined and not addicted to alcohol, smoking, pornography, money or any other harmful things. Do you fulfill these requirements? Please explain how.
